Denial percentage = currently denied claims divided by all filtered claims. Outstanding = billed − paid − contractual adjustments, floored at zero. Aging = elapsed calendar days from submission. Over-90-day A/R share = outstanding dollars on claims older than 90 days divided by all outstanding dollars. The fixed snapshot is September 1, 2026. This sample has no real payer policies, recoupments, secondary insurance, appeal deadlines, or first-pass adjudication history.
